Man boob disease procedure
- 1. Step: The patient is examined. Excess fat and breast tissue are identified. Blood and hormonal tests, if any, are evaluated.
- 2. Step: The patient is given general anesthesia in a hospital setting.
- 3. Step: A serum containing certain medications is injected into the area where surgery will be performed. Approximately 15-20 minutes is allowed for this (to help shrink blood vessels and obtain more refined fat).
- 4. Step: A small incision is made about four fingers below the areola, which is the brown part of the breast.
- 5. Step: The fat tissue around the breast is significantly reduced. Then, the entire mammary gland is removed.
- 6. Step: The incisions are closed with stitches, marking the end of the surgery.
- 7. Step: The patient can be discharged the same day or after one night and return home.
- 8. Step: Two days after the surgery, the patient will come to the clinic for a check-up and dressing change.
Attention : Dr. Ali Mezdeği performs his surgeries using the “3D gynecomastia” technique that he developed. He does not make an incision around the nipple. Additionally, he removes all the fat tissue in the chest area as an extra step, using liposuction. This results in a more aesthetic chest appearance.
Gynecomastia recovery day by day
- Day 1: A compression garment will be put on before you leave the operating room. There may be slight discomfort or pain.
- Day 3: The underpad placed beneath the compression garment will be removed, and the tightness will lessen. You will be allowed to shower and begin light arm stretching exercises.
- Day 14: Light exercise can be started. The intensity of the exercise should be gradually increased each day.
- Day 30: You will be allowed to swim in the pool or sea.
- Day 60: You can resume any pre-surgery exercise routines, even the most intense workouts. The surgical scar will begin to fade.
- Day 90 – 120: The surgical scar will match your skin tone and become barely noticeable.
Possible Complications:
- 1% chance of the scar not healing completely. This issue can be resolved with our recommendations.
- Early-stage hematoma may occur. If it does, it happens within the first 1-2 days and will be treated promptly.
- Late-stage seroma may occur. If it is seen, it usually happens within 10-15 days and will be drained with a syringe during check-ups.
- Numbness and loss of sensation in the surgery area, which will subside over time.
- Skin sagging or folding may be observed. Maintaining good posture and doing light exercise will help address this issue.
Pseudo Gynecomastia
Pseudo gynecomastia (Adipomastia) refers to the enlargement of the chest area due to fat accumulation caused by excessive weight gain. In general, exercise and diet can reduce the enlarged chest area. Additionally, sagging in the chest area can be observed in patients who lose a significant amount of weight after gaining excess weight.
Pseudogynecomastia Treatment
There are two treatment methods. Based on the patient’s examination findings, the chest can be flattened by removing excess fat through 3D Vaser liposuction alone. Alternatively, in addition to this procedure, the breast gland (glandular tissue) may need to be surgically removed.
Pseudogynecomastia Causes
The primary cause of pseudogynecomastia is fat accumulation due to excessive weight gain. It can also be observed when weight is lost rapidly after significant weight gain. Additionally, weight gain due to medications taken for other illnesses or a hormonal imbalance can also be a cause of pseudogynecomastia.
Pseudogynecomastia vs Gynecomastia
The only difference between the two is that in true gynecomastia, the swelling is due to hormone-related enlargement of the breast gland, which cannot be resolved through diet or exercise. The gland needs to be surgically removed, and the only solution is surgery.
In pseudogynecomastia, while it doesn’t happen for every patient, some may lose weight through diet and exercise. As the weight loss is balanced throughout the body, the thickness of the fat in the chest area may reduce as well. In this case, you can get rid of pseudogynecomastia without surgery. However, based on our experience, this is a rare occurrence, and most cases end up requiring surgery.
Gynecomastia Degrees
Grade 1 Gynecomastia
This is the mildest form of gynecomastia, typically characterized by only a small enlargement of breast tissue. In this case, there is no noticeable sagging or looseness of the breast tissue. Apart from the slight enlargement of the breast tissue, there are usually no other symptoms in 1st-degree gynecomastia, and it is often treated due to aesthetic concerns.
Grade 2 Gynecomastia:
This condition can often be treated with liposuction. Liposuction is used to remove excess fat tissue and can help reduce the growth of breast tissue. However, each case of gynecomastia is different, and treatment options may vary based on the individual’s situation. Removal of glandular tissue may be necessary.
Grade 3 Gynecomastia:
At this level, both fat accumulation and excessive swelling of the glandular tissue are observed. Typically, the glandular tissue (milk gland) is completely removed. Liposuction is also performed additionally.
Grade 4 Gynecomastia:
At this stage, the breast size is comparable to that of a woman’s breast. The solution involves removing the glandular tissue, performing VASER liposuction, and, in addition, excess skin is removed, similar to a breast reduction surgery in women. Removing the excess skin is essential, as leaving it can result in sagging and an undesirable aesthetic appearance.

Best gynecomastia surgeon in Turkey
In Turkey, Dr. Ali Mezdeği’s success rate for gynecomastia surgeries is over 99%. This rate refers to the success of the surgery on the first attempt without requiring a second revision surgery. This figure is based on his over 25 years of experience and the thousands of gynecomastia surgeries he has performed.
Reasons for his success include:
- The surgical incision is made 20 cm below the nipple, not at the areola. There is no scar around the nipple.
- The prevention of future breast enlargement by not only using liposuction but also completely removing the mammary gland (glandular tissue).
- The surgical incision is only 7-8 mm, and the surgery is completed through this small opening.
- Achieving a 3D appearance by completely balancing the fat distribution in the chest area, including liposuction extending almost to the armpits.
- The surgery is approached not just as a treatment but with a focus on planning for a more aesthetically pleasing chest.
FAQ
It is the name given to a surgery that not only addresses the issue of chest enlargement but also involves treating the entire chest area, extending even to the underarm, with the support of liposuction. Normally, the cost of 3D gynecomastia should be higher compared to the classic gynecomastia surgery, as the entire chest area is treated. However, we offer 3D gynecomastia to patients at the same cost. This provides two benefits for the patient: 1. The chest is reshaped, and 2. They get rid of the gynecomastia issue.
There is a 99.99% chance that fat accumulation will not occur again after gynecomastia surgery. We had a patient who gained 20 kilograms after the surgery. Upon examining this patient, we found that there was no fat accumulation, no breast enlargement, and no sagging in the breast. Therefore, I can confidently tell you that there will be no fat accumulation again after the surgery.
There is no upper age limit for gynecomastia surgeries. A patient can undergo this surgery even at 60 or 70 years old. As long as there is no illness that could prevent the surgery or the use of blood-thinning medication, it can be performed. For younger age groups, we can perform these surgeries starting from the age of 16-17, with parental consent and supervision.
There is no weight limit for gynecomastia surgery. In fact, setting a weight limit would not be appropriate. The most important factor is the patient’s examination findings. Even in a very thin patient with little or no fat, there can be an enlargement of the mammary gland, and surgery may be performed. Similarly, if a 120 kg, 1.80 m tall patient has suitable examination findings, they can also undergo surgery.
Many conditions worldwide can be treated with medication. However, gynecomastia is not a condition that can be treated with medication. There may be a small exception: if the cause of gynecomastia is a hormonal problem, hormone medications may temporarily reduce breast swelling. However, once the treatment ends, gynecomastia will reappear.
The easiest way to check if you have gynecomastia is to take off your shirt and look at yourself in the mirror. If you notice an enlargement in your chest area that shouldn’t normally be there, it might raise some suspicion. Additionally, flex your body like a bodybuilder. Gently touch your right chest with your left hand. If there is tissue moving over the muscle, indicating fat accumulation, then you likely have gynecomastia. This is the simplest method for an at-home gynecomastia test.
Yes, this was a problem that remained unsolved for years. However, with the 3D gynecomastia technique we developed after performing thousands of gynecomastia surgeries, we have completely eliminated this issue. After removing the breast glands, depression under the chest does occur, but if the depression is balanced with liposuction applied to the entire chest area, there is no issue after surgery. The likelihood of chest depression becomes almost zero. You should have no doubts about this.
Physiological gynecomastia is the name given to gynecomastia caused by the imbalance of estrogen and testosterone hormones in newborn babies, adolescents entering puberty, and older men. In all three of these stages, no medical intervention is necessary. Simply waiting is sufficient.
Revision gynecomastia surgery refers to a second gynecomastia procedure performed on patients who have previously undergone surgery but did not achieve the desired results. Gynecomastia surgery falls under the field of plastic surgery. The issue often arises when general surgeons, claiming to perform this surgery, operate on patients, leading to unsatisfactory outcomes. If you have a gynecomastia problem, we strongly recommend seeing a plastic surgeon.
- Hormonal Imbalance: A disruption in the balance between testosterone and estrogen.
- Side Effects of Medications: Side effects of certain drugs like antidepressants, heart medications, and steroids.
- Health Conditions: Conditions such as Klinefelter syndrome, hypogonadism, and tumors affecting hormone production.
- Lifestyle Factors: Excessive alcohol consumption, drug use, and obesity.
- Malnutrition: Prolonged malnutrition and starvation.
If gynecomastia is not treated, nothing will happen, friends. Gynecomastia is entirely a cosmetic procedure. If the size of your chest doesn’t bother you and you don’t have any issues with clothing, then there’s no need for surgery.
Gynecomastia is categorized into patients who have only fat tissue, those who have only breast gland tissue (glandular tissue), or those who have both, which we refer to as the mixed type. It is also classified based on the size of the male breast: 1st-degree gynecomastia, 2nd-degree gynecomastia, 3rd-degree gynecomastia, and 4th-degree gynecomastia. Men with very large breasts, resembling a female breast, are considered to have 4th-degree gynecomastia, and a traditional breast reduction surgery is performed.
It is certain that gynecomastia surgery is a cosmetic procedure. Any cosmetic surgery you are considering should be performed by aesthetic plastic surgeons. There is a boundary violation here. Do not trust doctors from other specialties who claim they can perform gynecomastia surgery.
Private health insurance or public insurance does not cover any plastic surgery procedures. Even in state hospitals, you may be required to pay the cost determined by the government.
Gynecomastia cannot be solved with exercise and diet. However, for patients who ask this question and have doubts in their minds, exercise and diet can be tried first. In our opinion, it won’t cure the condition, but it will remove any doubts. You won’t have to ask yourself, “Would it have gone away with exercise?”
Single breast gynecomastia is a very rare problem. What we commonly encounter in our cases is asymmetry. The two breasts can be so different that we may remove 600cc of fat from one and 1 liter from the other. However, in rare cases, gynecomastia can occur in just one breast. In such cases, only the affected breast is operated on, and the patient is satisfied with the result.
Unfortunately, gynecomastia does not go away on its own. If you have this issue and have already passed puberty, you cannot get rid of it naturally. The only solution to this problem is surgical intervention. Exercise and dieting will not resolve it. Additionally, as of 2024, there is no medication available to treat gynecomastia.
Sometimes, even after conducting all examinations (such as CT scans, lab tests, MRIs, and reviewing the patient’s medication history), we may not be able to determine the cause of breast enlargement. In such cases, we refer to gynecomastia with an unknown cause as idiopathic gynecomastia. Regardless of the cause, the solution remains the same. We resolve the gynecomastia issue with a short surgery.
Pathological gynecomastia refers to gynecomastia that arises due to an existing problem in the body. It is caused by issues that lead to an increase in estrogen hormone levels. For example, adrenal tumors, thyroid problems, or excessive fat tissue in the body can contribute to this condition.
A decrease in testosterone, the male hormone, also falls under this category. Examples include testicular cancer, diabetes, or medications used for stomach-related issues, as well as drugs used to treat depression, which can theoretically cause gynecomastia.
Athlete gynecomastia refers to breast enlargement observed in men who engage in bodybuilding. This condition is caused by the use of protein powders or similar substances that disrupt hormonal balance, leading to swelling of the breast glands in the chest area. Even if the substances are discontinued, the feminizing growth in the chest area may continue. We have many patients with this condition, and the issue is resolved through the application of 3D gynecomastia surgery for each patient.
The easiest way to distinguish at home is by flexing your body. With one hand, feel around your other flexed chest. If you feel round, firm tissue, it is likely gynecomastia. If there is fat spread around the chest and you are overweight, it could be what we call pseudogynecomastia, or chest fat.
Breast enlargement in adolescent males is often temporary. Hormonal imbalances are common during adolescence, so if estrogen levels rise slightly, breast enlargement may occur in a young male. If it doesn’t resolve by age 15, we recommend consulting an endocrinologist. If gynecomastia is diagnosed, you may undergo gynecomastia surgery in Turkey with parental consent.
